This 8 1/2" X 11", 50-sheet, two-sided pad has a chart on the front to capture information on meals, sleep time and more for each of your children. There's also a place for the babysitter to write notes to you. On the back is a medical release form and a list of items to review with the babysitter, such as where the fire extinguisher is located or where the phone is located on each floor.
